Distance provider when dialing The Black 411.  This charge has nothing to do with The Black 411 and may
be charged by your LD provider for calling an Atlanta Georgia phone number.  Your regular long distance
may apply.
By signing my electronic signature below, I confirm the information submitted & my authority to incur charges on the
telephone number listed above. I am 18 years of age or older & I understand & fully accept the terms & conditions of
this offer. I understand & agree that BFE Telecom Inc. & the Black 411™ will charge a one time $5.00 set-up fee to get
me subscribed to the service, which will be charged to my credit  or debit card.  The set up fee will not be charged to
your local phone bill and will be charged to a credit or debit car. I further authorize BFE to charge a monthly fee of
$5.00 to my local telephone bill for each month I use the service.  The Black 411™ service  will automatically  bill the
monthly service fee to my local telephone bill each month  I use the service. The charges will appear under the  
"miscellaneous charges & credits, or Enhanced Services" section of your telephone bill on behalf of BFE Telecom Inc.
& will appear as "The Black 411". The charges will be billed by ILD Telecommunications.   I understand that if I do not
use the service during any particular month I will not be charged any fees from BFE Telecom for The Black 411™

You have  the right to Dispute BFE Telecom Charges on behalf of  The Black 411™ billed on your local
telephone bill by ILD Telecommunications.  You are not legally responsible for The charges incurred by
minors or vulnerable adults without your consent.  Your local telephone service will not be disconnected
because you fail to pay a charge by  BFE Telecom on behalf of The Black 411™ except that non
payment of certain regulated telecommunication charges may result in disconnection of service in
Alabama, Florida, Georgia, Kentucky, Louisiana, South Carolina and Tennessee.
